Helpful Tips & Information
Type your answer for each question in a Word Document or Notepad first, and then copy and paste it into the online form when you are ready. In the event of a technical difficulty, having this backup means you won’t have to start from scratch. After you click the submit button at the bottom of the form, please ensure that you see a “Thank You” message acknowledging your submission. Google Chrome is the recommended browser for this form.
Review the program requirements, selection criteria and tentative class schedule, and ensure you can commit to attending within the specified time requirements should you be accepted into the program. All program days are from 8:00 a.m. to 5:00 p.m. Class members may miss a total of eight hours during the program. Sickness is an excused absence.
